Student-Athlete Conduct Statement

I hereby apply for the privilege of trying out for the St. Rose of Lima________________ team. I will conduct myself in a manner that will bring honor to my team, school, and community. I acknowledge that I will be requested to withdraw from the team for failure to obey the Code of Ethics as outlined below:

A.  AUTOMATIC DISMISSAL FROM SQUAD

1.       Violation of Rules and Regulations as outlined in the St. Rose Handbook

2.       Failure to maintain academic standards as determined by the school Principal

B.  AUTOMATIC DISMISSAL OR SUSPENSION FROM SQUAD

1.   Bringing discredit to team or school

2.   Destroying or stealing equipment or property

3.   Fighting

4.   Profane language

5.   Unauthorized absenteeism from school

6.   Acting in a disrespectful manner towards teachers, coaches, officials. teammates, opponents, fellow students and parents on and/or off the field

C.  GENERAL GUIDELINES

1.   Appropriate dress for school and athletic assemblies is required at all times

2.   Those desiring to drop from an activity must notify their coach in person

3.       Loss of any equipment issued by St. Rose must be paid by the individual

I have read the above rules and accept them as part of my responsibility for membership on any athletic team in St. Rose of Lima School
